How Patch Fitness uses the information you give when booking.

What we collect

Your name, email address and mobile number, your bookings and attendance, and, through the health questionnaire (PARQ), your date of birth, address, emergency contact details, answers to health questions and your signature.

Why

To manage your bookings, send you confirmations and reminders, and make sure it's safe for you to exercise (which our insurance also requires). We use your health information only with your explicit consent, which you give when you complete the questionnaire.

Who can see it

Only your coach at Patch Fitness. We don't sell or share your information. It's stored securely with our hosting and email providers, who process it on our behalf.

How long we keep it

While you keep booking with us, and for as long afterwards as our insurance requires. You'll be asked to complete the questionnaire again every 12 months.

Your rights

You can ask to see, correct or delete your information at any time, or withdraw your consent, by emailing george@patchfitness.co.uk. You can also complain to the Information Commissioner's Office (ico.org.uk).
